Valley Home Care is registering participants for its Diabetes Self-Management Program. Program topics include learning to live with and manage diabetes, disease process, monitoring and use of blood glucose results, nutritional management, medication review, changing behavior, goal setting, complications, foot care, exercise and community resources.
The program, held monthly, is a four-week course with two-hour meetings once a week. The next course dates are May 27, and June 3, 10, and 17. All meetings are held at 7 p.m. in the Kraft Center, 15 Essex Road, Paramus, NJ. Prior to participating, registrants must complete a one-hour individual assessment with a diabetes educator.
Many insurance companies, including Medicare, will cover this program as a benefit to their subscribers. The patient’s out-of-pocket cost will depend on the policy coverage and any limitations.

The American Diabetes Association believes that Valley Home Care’s program offers high-quality diabetes self-management education that is an essential component of effective diabetes treatment.
