Care Plus NJ Staff Recognized for Dedication in Working with Mentally Ill

The New Jersey Association of Mental Health Agencies recently awarded Care Plus NJ staff member Herlande Vilcant with Case Manager of the Year for the Northern NJ Region. Only two awards were given for the entire Northern NJ Region.

Vilcant is a member of Care Plus NJ's Integrated Case Management Services (ICMS) team. Herlande is calm under pressure, objective, compassionate and empathic. Her dedication to helping those struggling with mental illness is notable and she readily assists ICMS clients whenever a need arises. Herlande excels in finding Emergency Community Housing, as she is unwilling to see a client homeless on the street.

Integrated Case Management Services (ICMS) is an innovative program which provides a wide range of services to Bergen County residents being discharged from the county hospital and the state psychiatric hospital, as well as individuals with mental health issues living in the community. The overall goal of ICMS is to prevent hospital recidivism and assist client in achieving long-term wellness by helping them better manage their illness, increase their functioning and integrate them into the community. Services include supporting the discharge plan of the hospital, establishing appropriate linkage to mental health providers, utilizing community resources and advocating for clients. ICMS works with consumers in their home community environment through extensive outreach, advocacy, psycho-education, and supportive counseling.
