Medexi to Launch Innovative New Transitional Care Platform at HIMSS 2013

The Medexi organization announced today it will launch its Medexi 360 System, a highly-innovative transitional care platform, at the upcoming HIMSS Conference & Exhibition scheduled for March 3rd -7th in New Orleans, LA.
The Medexi 360 System is being billed as a comprehensive platform created to meet the needs of today’s evolving health care organizations by improving care outcomes and significantly reducing hospital readmissions by over 30% and greatly improving patient care satisfaction levels. The Medexi 360 System also leverages advanced SaaS technologies alongside evidence-based transitional care models to effectively manage care at patient discharge and beyond.
The platform has been designed and developed by health care professionals who have first-hand knowledge of the specific issues which contribute to hospital readmissions -- a growing concern for the health care industry as well as the federal government. It is estimated that 20% of Medicare hospital patients are readmitted in less than 30 days of discharge, translating to an additional $47 billion in hospital bills to be covered by American taxpayers. With the advent of the Hospital Readmissions Reduction Act (HRRA) implemented in October of 2012, hospitals with excessive readmission rates are now being financially penalized. It is projected that over 2,200 hospitals will be penalized in excess of $280 million in 2013.
Medexi 360 key features include:
-real-time alerts sent directly to physicians on potential 30-day readmission patients to advise of issues thereby enabling immediate action
-populated dashboards providing; a single view for a patient throughout the continuity of care, includes history of present illness, labresults, radiograph interpretation, prescriptions, and transitional coach communications
-transitional care coach module which includes an in-home patient visits within 24-hour period followed by a 30-day monitoring scheduleto ensure patient compliance and positive care outcomes
Medexi’s innovative platform has been well received by the health care industry and viewed as a viable solution to reducing possible Medicare penalties. “With the implementation of the Medexi 360 System, we can better manage the transitional care process and are better positioned to negate risk and preserve our bottom line”, said Tony Marinello, CEO – Mountain Vista Medical Center.
Dan Deublein, Medexi’s Founder and an Emergency Room Physician Assistant, will be attending the HIMSS show and available to meet with hospital administrators and personnel. “I see the inherent weakness within traditional patient discharge models which ultimately leads to sub-par patient outcomes. By creating an integrative view of a patient, we have bridged the information gaps which lead to seamless transitions from one provider to the next”, said Deublein.
Medexi representatives will be available at booth #2569 throughout the duration of the show. Appointments may be scheduled in advance by calling 800.325.7103 or visiting www.medexi.com.
